AI Doctor App?

Good afternoon, everyone! I am super excited about the possibilities that AI is opening up in HMIS and healthcare. As a new FNP, I feel the gravity of my assessments, diagnoses, and treatments for all patients, even those with straightforward problems. I still consult my texts and guides for every case, and double and triple-check my pharmaceutical prescriptions. I remember learning that 76% of diagnoses could be obtained from a patient’s subjective data alone, so I spend time carefully collecting as much information as I can during the health history. This is easy for me to do because my FNP practice is a side-line gig, something I do as a ministry for friends and family as I rely on my full-time, low-stress job as a home health nurse to pay my mortgage. But in most clinical practices, primary care providers see one patient every 20 minutes and write more than 20 prescriptions per day, with an average visit count falling between 15 to 25 patients per day (Wolters Kluwer, 2020). With that lofty number it is just not possible to research much on each patient, and errors are not unlikely to happen in this heavy workload environment.

But what if artificial intelligence (AI) did the heavy hitting for us? As office staff, nurse aids, nurses, and phlebotomists go about their business performing ancillary healthcare services gathering patients’ demographic data, subjective information, vital signs, and lab work, instead of merely typing it into a typical software database, this information could interface with an AI platform which specializes in robust medical diagnostic systems. These AI systems are also called large language models (LLMs). They are machine learning systems that produce humanlike responses from language, and can solve complex cases, show clinical reasoning abilities, take patient histories, and even display empathetic communication (Savage et al., 2024). The AI platform could then function as the brains of the provider and ‘crunch’ the data into a viable diagnosis. From here, the treatment plan would logically flow, along with the usual prescriptions, further tests, and referrals. It is mind-boggling. But could an AI function as well as a real human medical provider?

According to this provocative study in JAMA by Goh et al. in 2024, AI could–and did–outperform medical providers. This study was a randomized clinical trial, and it was conducted over one month in 2023. It investigated whether physicians with three years average experience with training in family medicine, internal medicine, or emergency medicine were more successful diagnosticians when they utilized AI tools (the intervention) versus traditional tools. Surprisingly, they were not. The physicians that used ‘conventional’ research aids such as UpToDate, Google, and textbooks, were 74% accurate in their diagnoses, and the physicians that used AI-enhanced search tools of ChatGPT Plus [GPT-4] and OpenAI were only 76% accurate, which was not a statistically significant improvement. However, the LLM alone was 92% successful in finding the correct diagnosis (Goh et al., 2024). Because the researchers’ original hypothesis was whether AI intervention helped in providers’ diagnostic awareness, and were not looking at AI versus providers, they did not heavily interpret or extrapolate these incidental findings, even though they did state that more research is necessary in marrying these two entities: Physicians and AI tools.

No one wants to put himself out of a job, even if AI is showing promise of being a better care provider than a physician. In light of this admittedly small study, I envision that AI will be the future answer to our broken healthcare system and that it will bring an open-source mindset into the highly political and powerful arena of medical information, which is modern medicine.

Syringa (Lilac) is a genus of 12 currently recognized species of flowering woody plants in the olive family (Oleaceae), native to woodland and scrub from southeastern Europe to eastern Asia, and widely and commonly cultivated in temperate areas elsewhere.  

And they smell so lovely!

Essential Oils

This is a picture of my current collection of essential oils.  I have posted about them before but thought an update would be fun!  I like Young Living essential oils because they are super pure.  Gary Young, the founder of the company, was a lumberjack who sustained an injury many years ago, and in his quest for cure, he discovered the healing properties of essential oils, bought farms to ensure the highest quality herbs, and began distilling them, creating this company.  Young Living is now a huge and thriving business serving the world with the purest essential oils you can buy.

I use them every day.  I use them for spiritual reasons, for my perfume, for restorative properties, for curing illnesses, and as daily protection against pathogens and cancer.  I use them straight on my skin, in carrier oils, and in capsules which I take by mouth.  You can take all Young Living essential oils by mouth (but you must be careful because some oils are very hot, and can burn your skin).

Most people use a few drops of essential oils in their bath or a diffuser, which is wonderful because the tiny therapeutic particles are released into the air and when they enter your lungs and brain (via the olfactory area), they begin healing you immediately at the cellular level.  Although it is referred to as ‘aromatherapy’, the use of essential oils is far from being about scent.  It is plant medicine at its most intense and pure level.

Milk

And I am come down to deliver them out of the hand of the Egyptians, and to bring them up out of that land unto a good land and a large, unto a land flowing with milk and honey…~Exodus 3:8

Milk and honey were considered blessings in the ancient times so much so that it was embedded in the promise of God in delivering His people from our captivity.  Milk and honey are excesses that are not needed, but that make life all that much more special.  Think of all the products made from dairy and it becomes obvious.  Man has not found the ability to make milk for consumption, and he never will.  Man has found ways, through evil forces, to tamper with this pristine gift from God, give it a bad rap, and lie about its unique attributes.

I am a lover of milk and I am here to thank God for this blessed gift!  As with many or most gifts from God, man has adulterated milk to the point of being downright unhealthy.  I, like most Americans, have grown up drinking processed and pasteurized milk, which is unfortunate because I have drunk so much milk and eaten so many dairy products in my lifetime thus far, that I undoubtedly have missed out on much of the health benefits I would have otherwise derived from consuming fresh, raw, or unprocessed milk.

I recently have begun receiving more and more blessings from God.  One of them has been, through a good friend, to be able to receive fresh cow’s milk from a Jersey cow.  It is unsurprising that man has made drinking this primary blessing and gift from God a federal offense!  Like so many other things given to us by God, in nature, it is a federal offense to sell unpasteurized milk for human consumption.  I realize this likely stemmed from some real bad cases where people died from drinking raw milk, historically.  After all, harmful bacteria and chemicals can be harbored in raw milk, just as with any other warm biological liquor.  Abraham Lincoln’s own mother died from drinking raw milk because the cow had eaten something poisonous.  But there should be a caveat in the law for those of us who understand the risks but want to purchase and consume it anyway.

Cows are ‘freshened’ by their handler every year or so when they birth a new calf.  When a mammal undergoes parturition, the mammary glands are stimulated to produce milk.  The udders of a cow are four, and they become engorged just like a human female’s do after she births.  My friend’s Jersey cow just calfed and is now giving 7 gallons of milk per day!  The calf is removed from her mother and given milk on the side so that the farmer milks the cow straight into stainless steel buckets, twice per day, morning and night.  Farmers are very careful about using very clean, stainless steel utensils because any additional or foreign bacteria will cause milk to spoil much faster.  In our days of ultrapasterurization (heating to high degrees to kill all bacteria), we are getting used to our milk products having a shelf life of a couple weeks.  However, in this process of pasteurization, the same heat that kills the potentially harmful bacteria also kills the good stuff as well (see article highlighted above).  Fresh milk handled carefully can last 5 to 7 days.

I got my first gallon of fresh milk yesterday when I drove out to her farm.  It is very satisfying to get your food from a place and from people you knew grew it or made it.  This is the reason farmer’s markets are so popular.  There is accountability on the part of the farmer, and responsibility on my part because we are both participating in an endeavor whereby we give our stamp of approval, without needing the FDA to pass judgment.

We are made to grow our own food and work the land.  My grandparents’ generation was pretty much the last to do this in America, so we and our parents are used to purchasing our consumables at a grocery store.  It is obvious that the distances traveled and separation involved in obtaining our food in this way is inferior to knowing your farmer and your farm.

It dawned on me this morning that most people have no idea what fresh milk looks like, so I took a picture of how it looks.  Once the milk is obtained from the beast, the farmer carries it inside and filters it with a stainless steel sieve.  This removes particles of hair or dirt that might have gotten into the milk.  After all, animals are hairy and live in field and barn.  The farmer then pours the milk into gallon, half gallon, or other size jars and begins cooling it down (on ice or eventually in the refrigerator), because it is body temperature when milked.  As the milk sits, the cream begins to rise to the top.  The amount of cream depends upon many conditions and is extremely variable depending upon the breed of cow, days since parturition, food fed, milking frequency, and stress of the animal.

Commercial dairy farmers and milk production facilities control for the amount of fat in the milk by pouring off the milk fat and adding back in the amount desired, like 2%, 4%, etc.  Or you can buy straight cream for whipping at 30%.  The excess milk fat is then used to make dairy products, which is why dairy products can be expensive:  they require a lot of cream which requires a lot of milk, versus just drinking the milk liquor, or watery part of milk.  The cream is dense and opaque which is why skimmed milk is more clear throughout.

Sometimes the jar of a fat rich milk breed like a Jersey will have nearly half the container be cream!  Right now, since this cow just freshened and she is producing large quantities of milk, she has a higher water to fat ratio, so her milk is not quite a rich, cream-wise.  I had about 2 inches of cream on this gallon jar of milk yesterday.  I poured off some of the cream to use for my coffee, but am leaving the rest to drink as whole milk.  If I am fortunate to be able to keep buying her milk, as the cow’s supply diminishes, the fat to water ratio will increase, possibly giving me enough cream to pour off to make butter or ice cream.  🙂

As you can see, I have already drunk a fair amount of my fresh milk.  It has a bit of a chalky texture that takes some getting used to when you have been raised on store milk which is more silky.  I attribute this to the nutrients that are active and floating throughout the milk.  The taste is full and earthy versus flat and clear as with store milk.  My 7 year old loves it as well, if that is any testament to its deliciousness~

His Blessings: Chocolate!

Though life can be hard for Christians and we suffer persecutions, tribulations, reproaches, infirmities, insults, and distresses, He doesn’t leave us without numerous blessings which He bestows on all men, gifted to us in this earth and the things within and upon it.  I would like to honor Him for these natural substances that we usually take for granted, one of my favorites being

CHOCOLATE!

Chocolate grows in large pods on trees native to Central and South America.  They are knocked off the tree and cut open to reveal cocoa beans inside, which are then fermented, dried, roasted, and ground into cocoa powder.  Usually sugar is added and maybe cocoa butter to make a fine bar of chocolate.  How finely chocolate is ground, and the quality of the ingredients is usually what separates the best chocolate from mediocre chocolate because the human tongue can detect even the minutest textural characteristics, in other words if your tongue feels tiny granules of chocolate from a coarser grind, it is not as delightful as if your tongue feels silky smoothness.

Chocolate is also healthy for us, as this article reports.  Finally, one of the best movies ever is entitled Chocolat.  I highly recommend it!

Thank you Lord GOD for giving us the fruit of the cacao tree.  It is not food forthe gods but food from GOD.

Bizarre Behavior (i.e. Schizophrenia) and Sin

Since the advent of philosopher-physicians in ancient times, man has had millennia to develop his manuals of medical disorders. Rene Descartes, a philosopher and mathematician of the 17th century, created the paradigm of mind/body duality, and this ideology has infiltrated every modern discipline of behavioral and physical science. Behavioral problems are now understood by the masses to fall under the domain of medical disease, known as ‘mental illness’; and their cause is attributed to brain chemicals gone awry.

Terms for these diseases are constantly changing as evidenced by the frequent revision of the DSM (Diagnostic and Statistical Manual of Mental Disorders) specifically its definitions and diagnoses. The DSM is the psychologists’ bible, first published in 1952, and has undergone 5 official revisions since then.

If science can prove that behavior problems are rooted in brain chemicals, and especially genes, then it would absolve man from the bulk of his responsibility for his behavior as he could then blame his diseased brain, versus his faulty mind; his brain chemicals versus his sinful lifestyle. Then simply take a pill to fix himself.

If science could find those biochemical pathways that show chemicals go askew causing bizarre behavior, inventors could then make drugs for someone to take to moderate the symptoms of these disorders, which they could call ‘treatment’. This would be analogous to how they develop drugs to moderate symptoms of physical diseases, such as high blood pressure or high blood sugar.

But are drugs really an effective treatment? Mental health professionals readily admit they are not curing the disease. Indeed, our best mental scientists–all who dabble in the realm of the psyche–consign you to the purgatory of lifelong affliction and force you to believe that you cannot be cured. The best THEY have to offer is daily or weekly medication; stop taking this medication and you will immediately relapse into psychosis or worse–criminal behavior. Stop taking your medication and you–and those around you–can expect utter DOOM.

Just in the last century, these diagnoses have become part of the linga franca of all ‘developed’ countries. Schizophrenia, bipolar disorder, and narcissistic personality disorder are just some of the trendy terms we have all become acquainted with in popular culture. But how much scientific evidence is there that shows these disorders are rooted in biology? I mean, if the DSM has been around for 70 years, that’s a lot of time for research studies to be conducted. I am pretty sure the direct link between behavioral problems and brain chemistry is still tenuous at best. But if scientists could eventually show a direct link to biological brain changes, how do we know this altered biology could not itself be attributed to simply…. sin?

For those familiar with bible stories, there are two notorious examples of kings who exhibited bizarre or erratic behaviors we would probably label depression, bipolar disorder, even schizophrenia, and definitely narcissistic personality disorder! For example, King Saul in the Old Testament experienced intense mood swings, episodes of deep depression, and erratic behavior. He hated David intensely, then had overwhelming feelings of love toward him. Another example is King Nebuchadnezzar, who suffered a period of madness where he lived like an animal for seven years, crawled around naked on all four limbs, and even ate grass!

But we also know that these kings had grievously sinned against God by setting themselves up as idols, which is the worst sin of all. And God gave them many opportunities to repent and turn back to him, which they denied. God gave them His most anointed soldiers, namely David and Daniel, to be personal assistants and intimate companions of the kings yet they tried to kill them. Eventually, Nebuchadnezzar did repent and turn back to God, but King Saul died in his sin, along with his sons.

A google search from today shows that research might indicate that those with symptoms of the disorder known as schizophrenia have a unique brain network, and cognitive difficulties related to attention and memory. And bipolar disorder seems to reveal reduced gray matter volume in regions such as the prefrontal cortex and hippocampus, crucial for cognitive control, impulsivity, and emotional regulation. The neurotransmitters typically studied are noradrenaline, dopamine, serotonin, and GABA, along with all the hormones; basically any liquor of the body might be implicated.

Although there are many studies on the neurobiological basis of bipolar disorder, its pathophysiology is not fully known. Studies indicate that all brain regions are structurally and functionally affected in bipolar patients. 

What will it mean if men are able to prove, through the scientific process, beyond a reasonable doubt, that there are abnormal biochemical pathways in the brain? For scientists and those who practice medicine, it will mean the cause of personality problems and bad behavior is organic, and that is enough information for them to assure you that it isn’t your fault, that your brain chemistry is out of whack. They will consider this a grand achievement and move on to their primary aim, which is to develop medications you need to take to moderate your neurotransmitters and hormones, so that you can gain control over your problem. These medications make drug developers rich and pacify healthcare providers because they can quickly and easily treat you with the stroke of the pen or keyboard key. Contrary to what they want you to think, these medications are not sophisticated at all. They usually work in one of two ways: to depress or stimulate you.

But what if those very brain chemicals were altered by God for a reason? What if God allowed your neurotransmitters and hormones to be out of whack because He is punishing you for your disobedience and sinful thought processes or lifestyle, and in order to get your attention to save your soul? Is this outside the realm of possibility? Let’s look back at the symptoms of these popular diseases listed above: Schizophrenia seems to create new neural networks affecting thinking, memory, and attention. And bipolar disorder causes major shifts in the parts of your brain that cause you to be able to think and control your emotions. This means collectively that you won’t be able to think straight or control yourself very well. Hmm. This kinda sounds like what the bible says happens to fools who sin against God and who do not obey Him. Bad stuff happens to them:

The way of the transgressor is hard. ~Proverbs 13:15

God is far away from the sinner. ~Proverbs 15:29

God does not honor the prayers of an unrepentent husband. ~1 Peter 3:7

Now the Holy Spirit tells us clearly that in the last times some will turn away from the true faith; they will follow deceptive spirits and teachings that come from demons, through the insincerity of liars whose consciences are seared. ~1 Timothy 4:1-2

God turns them over to their reprobate minds (Romans 1:28)….. and delivers such a one to Satan for destruction of the flesh, so that his spirit may be saved in the day of the Lord. ~1 Corinthians 5:5

If God can bring serial plagues on Egypt for their idolatry, could He not also tweak some liquid bodily humors and brain synapses to make you feel bad, to maybe get your attention and the attention of those around you? And then turn you over to the devil to help you realize the error of your ways, in order to turn back to Him? I think the answer is a resounding YES, He would! For further scriptural proof of this, I will cite the Apostle Paul’s letter to the Romans:

God’s Wrath on Unrighteousness

For the wrath of God is revealed from heaven against all ungodliness and unrighteousness of men, who suppress the truth in unrighteousness, because what may be known of God is manifest in them, for God has shown it to them. For since the creation of the world His invisible attributes are clearly seen, being understood by the things that are made, even His eternal power and Godhead, so that they are without excuse, because, although they knew God, they did not glorify Him as God, nor were thankful, but became futile in their thoughts, and their foolish hearts were darkened. Professing to be wise, they became fools, and changed the glory of the incorruptible God into an image made like corruptible man—and birds and four-footed animals and creeping things.

Therefore God also gave them up to uncleanness, in the lusts of their hearts, to dishonor their bodies among themselves, who exchanged the truth of God for the lie, and worshiped and served the creature rather than the Creator, who is blessed forever. Amen.

For this reason God gave them up to vile passions. For even their women exchanged the natural use for what is against nature. Likewise also the men, leaving the natural use of the woman, burned in their lust for one another, men with men committing what is shameful, and receiving in themselves the penalty of their error which was due.

And even as they did not like to retain God in their knowledge, God gave them over to a debased mind, to do those things which are not fitting; being filled with all unrighteousness, sexual immorality, wickedness, covetousness, maliciousness; full of envy, murder, strife, deceit, evil-mindedness; they are whisperers, backbiters, haters of God, violent, proud, boasters, inventors of evil things, disobedient to parents, undiscerning, untrustworthy, unloving, unforgiving, unmerciful; who, knowing the righteous judgment of God, that those who practice such things are deserving of death, not only do the same but also approve of those who practice them. ~Romans 1:18-32

The Christian has more to offer than pharmaceutical prescriptions. The Christian gives HOPE for problems of the mind, or mental disease!

Because these ‘diseases’ are not rooted in faulty brain chemicals at all, but they are rooted in SIN. And SIN has a cure. That cure is free, and His name is Jesus.

My Ministry Has Begun!

I have now successfully conducted three home health nurse practitioner visits and I am very pleased with the outcome and the satisfaction I feel doing this ministry for my Lord. I am still working out some kinks such as in getting to know my electronic charting system, but that is to be expected. As directed by God I do not charge for my services, but have received a cash offering thus far, which is much appreciated! My visits last about 45 to 60 minutes which is just right.

It has been confirmed to me that health care in America has been unnecessarily complicated by several factors which I would like to contemplate here. Possibly the weightiest variable is that people have become patients. A patient implies passivity, as in patiently waiting to be practiced upon. What I mean by this is that there is a psychology inherent in our society that we must be well, without pain, and without illness; and if we are not well, we merely need to present ourselves to a local doctor, submit ourselves to him passively, and become fixed. If we do not get cured, then that means there is a breakdown in access to medical care, whether that is physical, mental, or financial. That illness, pain, and suffering could be part of our human reality is rejected by us, and if we cannot be made well, we blame the ‘medical system’. Since society is made up of persons, each person comes in with the premise that

I deserve to be well and if I am not well, that is society’s [doctor, hospital, insurance, government] problem to solve ~

We feel that being healthy is a right. And we blame doctors and hospitals when we access care and we are not fixed, and not fixed immediately! We blame doctors and hospitals when we are charged too much or when the cure fails, as it inevitably will. Why? Because doctors are not God, and sickness and suffering are part of the human condition.

Is being healed biblical? Yes, it is. In the bible, doctors as such are rarely mentioned and when they are mentioned it is usually in derision, or in the context of a waste of time and provisions. Who is the Healer in the bible?

Jesus

Time and again I see good people, Christians even, purchase sophisticated tests and spare no expense or effort to be made well. There is no cure they will not travel to the clinic to receive, even if they do not have the means or transportation. They are affronted at the price of healthcare and offended at the thought of actually having to pay for it. People work so hard to obtain what they perceive as medical care for any and every symptom. They are willing to take any testing even if it involves huge amounts of radiation as with CT scans, and more and more medications each year to modulate their body’s response to life, circumstance, heredity, or lifestyle choices. Then after combing through their body with high-powered scopes and unnatural means, when they receive the fateful diagnosis of cancer or other disease, they become fraught with fear over how they are going to deal with this news. It becomes their focus, and by default becomes their families’ and friends’ focus as well. In biblical terms, it becomes their new idol. God did not intend for us to scrutinize our bodies to that degree! Our eyes are not made to penetrate the depths of cells or inner flesh, nor minds. So, it should be no surprise that we cannot handle the information we receive when we do. It is just another tactic of the devil to give us more power than we should have access to.

Christians have been duped–right along with everyone else–to demand healthcare in the form of western medicine. An example is how many flocked to get Covid vaccines and wear masks due to fear of dying from Covid-19, a novel Coronavirus. The average elderly patient in home health care takes about 5 to 10 different prescription medications. If you know me, you know that it is a pet peeve of mine that Christians give lip service to Jesus until they get sick, then they go to their false god in a white coat for their ‘real cure’. I realize they know not what they do, and are honest victims of the anomaly known as ‘medical care’, but come on, Christians should know better!

To try and deal with this petulant demand of American citizens, systems have been formulated and manipulated to address every possibility for a patient encounter, complaint, lifestyle choice, symptom, risk factor, and even gender ideology! This has occurred to such a degree that Medicare decrees how a literal office visit must go, how many minutes it should last, what tests must be ordered, what vaccines encouraged, and what medications prescribed. If you deviate from their algorithm at all, you can not only not receive payment, you will likely be audited, but you could also be investigated by the state medical board! Again, we all saw this happen (unlawfully) during Covid when some physicians who refused to treat according to the mandated narrative at the time, lost their licenses, or were even jailed! The type of care this medical model fosters is one of defensive medicine. A care provider is deemed good if they are simply following the algorithm, and bad if they are practicing outside the majority-approved doctrine. That is why many doctors are leaving ‘the system’ and going off on their own and not taking Medicare payments.

But unfortunately, it isn’t that easy to escape the clutches of the model of western medicine, because its pervasive reach is rooted in the fundamental ideology that people believe they deserve to be well, and to be made well.

I envision my ministry as healing those God sends to me in the way He decrees to me. This might be as simple as listening to their story and validating their experiences. It could be listening to their physical ailments and helping them troubleshoot healing modalities they might not have tried. It might involve prescribing medication that can treat an ailment with the goal of fixing the underlying problem, if possible. It might involve laying on of hands, anointing with oil, and praying for a cure. It might even involve casting out demons. It might involve doing nothing instead of doing something than can make things worse.

Regarding their health, people can be like children when they demand a smorgasbord of treatments and have a temper tantrum if they do not receive the perfect anecdote to what ails them. I witness this every day in my job. But that is God’s plan for us here on earth. God has allowed satan to run this world and there is no guarantee we will live a healthy or happy life. Saint Paul of biblical fame had a thorn in his side that God would not cure even though he begged numerous times for relief. The Apostle Paul came to see his ailment as a gift, as a sign to him and others that his strength came from not himself, but from our Savior, Jesus Christ, lest be become too proud in his flesh.

People are proud and demand to be made well. Pride is one of the seven deadly sins. Where pride is, humility cannot exist. And we are called to be humble servants of our Lord God. Sometimes God allows us to suffer, to be in pain, and to have an ailment. But the bible says after a while, He will deliver us from that same suffering, but we must have faith and believe in Him for this.

So, to tie up my thoughts here, God’s people are like children, we want everything to be perfect in our lives, including our health. To obtain this end, we demand that society provide this to us in a way that doesn’t cost us too much. We don’t look to God for His plan and His cure in His way and His timing. We make care providers our false gods and our hospitals temples for false gods. When these things inevitably fail, we become infuriated and blame them. Then at some point when we reach the end of ourselves, we feel abandoned by God. The irony is that we abandoned God to begin with, by not trusting Him fully. God isn’t just a deity we sing to and pray to in name (or in vain) on Sunday. He is our biggest Fan, our greatest Lover, and our only Healer. We must look to Him first and foremost for our healing and well-being, for everything.

And that is who I serve in my practice: God. I am so thankful I have been granted standing orders from God to treat His people in His way ~ ❤
